The registration fee is $75 and pre-registration is required. Registration includes a full day program of speakers and panel discussions, the book Strategies for Cultural Competency in Indian Health Care, the DVD Creating Space, and continental breakfast, lunch, and afternoon break refreshments.

Register by mail by printing this conference registration form. Complete and send it with your check for $75 to the address on the form. Confirmation of your registration will be sent by e-mail within two weeks of receiving your registration and payment.
